Job Opportunity: RN - ICU (Illinois, Silvis)
Position Details

Required Experience

Health and Safety Requirements
- Fit Test: Required
- Drug Screen: Trinity 10 Panel Drug Screen completed within 30 days of start (includes multiple substances; no THC)
- COVID-19 Vaccine: Proof of booster by September 2025 required; declination not accepted in Illinois
- Influenza Vaccine: Required for 10/1/2025 - 4/30/2026 season; declination not accepted in Illinois
- MMR (Measles, Mumps, Rubella): Proof of 2 vaccinations or IgG titer required
- Hepatitis B: Must have started vaccination series prior to start; titers not accepted
- Varicella: Proof of vaccination or IgG titer required
- Tdap: Current vaccine required; declination accepted only if no pediatric interaction
- 2 Step PPD OR IGRA (Quantiferon/Tspot): Required; chest x-ray if positive TB per regional guidelines
- Physical/Health Statement: Documentation within last year confirming fitness to work
Professional and Compliance Requirements
- Trinity Profile: Resume, skills checklist (within 1 year), and 2 professional references required for submission
- Trinity Code of Conduct Form: Must be signed and on file
- Trinity I9: Standard I9 completed by agency
- Confidentiality and Security Agreement: Must be signed and on file
- Integrity and Audit Services Form: Signed prior to start
- Non Employee Upload Form: Completed and uploaded
- Specialty Specific Competency Testing: Copy of specialty exam completed within last year required
- Color Vision Test: Required
- Child and Adult Dependent Abuse Training: Required (Iowa only)
- Conflict of Interest Disclosure Form: Must be signed and on file
- Criminal Background Check: Federal, nationwide, county checks for last 7 years; SSN verification; dated within 30 days of start
- Education Verification: Highest academic degree or diploma/GED verified
- EPLS (Excluded Parties List System) Check: Within 30 days of start
- GSA/OIG Exclusion Check: Within 30 days of start
- Iowa IA Sing Report: Required for Iowa contractors only
- Joint Commission CORE Exam: Within last year
- OFAC Sanctions Search: Within 30 days of start
- NSO (National Sex Offender) Check: Within 30 days of start; includes statewide checks for Michigan
- System Access Form: Completed and uploaded one week before start
- Skills Checklist: Provided by agency for clinical contractors
This position requires a well-prepared ICU RN with current certifications, comprehensive health screenings, and compliance with multiple professional and legal requirements. Prior travel experience is not required.
