Position Details

• Drug Screen: Trinity 10 Panel Drug Screen completed within 30 days of start (includes multiple substances; no THC)
• COVID-19 Vaccine: Proof of booster by September 2025 required; declination not accepted in Illinois
• Influenza Vaccine: Required for 10/1/2025 - 4/30/2026 season; declination not accepted in Illinois
• MMR (Measles, Mumps, Rubella): Proof of 2 vaccinations or IgG titer required
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted
• Varicella: Proof of vaccination or IgG titer required
• Tdap: Current vaccine required; declination accepted only if no pediatric interaction
• 2 Step PPD OR IGRA (Quantiferon/Tspot): Required; chest x-ray if positive TB per regional guidelines
• Physical/Health Statement: Documentation within last year confirming fitness to work

Professional and Compliance Requirements

• Criminal Background Check: Federal, nationwide, county checks for last 7 years; SSN verification; dated within 30 days of start
