Job Details

• Work schedule includes every third weekend and every other holiday rotation.

• Influenza Vaccine: Proof of vaccine, declination or exemption accepted except for CA or IL (valid for 10/1/2024 - 4/30/2025).
• MMR: Proof of 2 vaccinations or IgG titer.
• Hepatitis B: Must have started vaccination series prior to start; titers not accepted.
• Varicella: Proof of 1 vaccination or IgG titer.
• 2 Step PPD OR IGRA (Quantiferon/Tspot): TB screening required; positive TB requires chest x-ray per state guidelines.
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work.
• Tdap vaccine: Current, non-expired required; declinations accepted if no pediatric interaction.

Professional and Compliance Requirements

• Trinity Code of Conduct Form (signed).
• Trinity I9 form.
• Confidentiality and System Security Agreement (signed).
• Integrity and Audit Services Form (signed shortly before start).
