Job Opportunity: Respiratory Therapist

Location: Mason City, Iowa
Employment Type: Long-Term Assignment (LTA)
Contract Length: 12 weeks

Work Schedule

• Shift: 7:00 PM – 7:30 AM
• One holiday shift if a holiday occurs during the contract
• No Return to Origin (RTO) approved in March
• Every third weekend required

Required Qualifications

Licenses & Certifications

• Valid Respiratory Therapist license for Iowa
• BLS (Basic Life Support) certification (American Heart Association)
• ACLS (Advanced Cardiovascular Life Support) certification (American Heart Association)
• License/Certificate verification dated within 30 days of start
• Fit Test
• Specialty exam completed within the last year

Health & Safety Requirements

• COVID-19 vaccine (booster by Sept 2025) or approved exemption/declination (declination not accepted in CA or IL)
• MMR vaccination proof or immunity
• Hepatitis B vaccination series (proof required; titers not accepted)
• Influenza vaccine or approved exemption (for 10/1/2024 - 4/30/2025 season)
• 2 Step PPD OR IGRA (Quantiferon/Tspot) test
• Varicella vaccination or immunity proof
• Physical/Health statement confirming fitness to work (within last year)
• 10-panel drug screen completed within 30 days of start (no THC)
• Tdap vaccine (declinations accepted if no pediatric interaction)

Professional & Compliance Requirements

• Signed Trinity Code of Conduct Form
• Trinity I9 form completed by agency
• Confidentiality and Information System Security Agreement signed
• Integrity and Audit Services Form signed prior to start
• Non-Employee Upload Form completed
• Criminal Background Check (Federal, Nationwide, County, last 7 years) dated within 30 days of start
• Education Verification (highest academic degree or equivalent)
• EPLS (Excluded Parties List System) check dated within 30 days
• OIG/GSA exclusion check dated within 30 days
• Joint Commission CORE Exam within the last year
• National Sex Offender Registry check dated within 30 days
• System Access Form completed and uploaded one week before start
• Skills Checklist completed within the last year
• Child and Adult Dependent Abuse Training (Iowa only)
• Iowa-specific Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check within 30 days prior to start
• OFAC Sanctions check completed within 30 days prior to start
