Job Description

• Work hours: 8:00 AM – 4:30 PM
• Required certifications: BLS, ARRT License

Required Qualifications

• License: Valid Radiation Therapist license (ARRT)
• Certifications:

  • Basic Life Support (BLS) certification from the American Heart Association
• Experience:

  • Minimum years of experience in Radiation Therapy: Not explicitly stated; assume standard industry requirement

Health & Safety Requirements

• Proof of COVID-19 vaccination or approved exemption (booster required through September 2025)
• MMR vaccination proof required

• Influenza vaccine or approved exemption required (valid for flu season 10/1/2024 - 4/30/2025)
• TB screening: 2-step PPD or IGRA test required; positive TB requires chest x-ray within last year (Michigan-specific)
• Varicella vaccination proof required
• Physical/Health statement confirming fitness to work (within last year)
• Tdap vaccine required (declinations accepted if no pediatric interaction)
• Respirator evaluation with physician signature required

Screening & Background Checks

• 10-panel drug screen (no THC) completed within 30 days of start date
• Criminal background check (federal, nationwide, county) within last 7 years, including SSN verification
• Education verification (highest level of academic education)
• Exclusion screenings: EPLS, OIG/GSA, NSO (including Michigan statewide check)
• Conflict of Interest Disclosure and Confidentiality Agreements required
• Integrity and Audit Services form signed prior to start
• Code of Conduct form required
• I9 verification completed by agency

Testing & Competency

• Specialty-specific competency testing completed within the last year
• Skills checklist completed within the last year
• Joint Commission CORE exam within the last year
• License/certificate verification within 30 days of start
• Trinity system access form completed one week before start

Additional Notes

• All documentation and certifications must be uploaded to the agency’s system prior to onboarding
• Declinations for vaccines accepted only with agency letterhead and where allowed by state regulations (not accepted in CA or IL)
