Travel RN - Case Management

Job Summary

Seeking a Travel Registered Nurse (RN) specializing in Case Management to join a Level II Trauma Center hospital. The unit primarily operates day shifts with 8-hour shifts, five days a week. No night shifts are required.

Unit Information

• Bed Capacity: 290 beds
• Staffing: Need 12-13 RNs for Case Management to cover a full census
• Patient Ratios:

  • Case Management: 1:25 on floors, 1:20 in UC units
  • Social Work: 1:30 in ICU, 1:40 on floors
• Equipment: iPad, computer, cell phone
• EMR Systems: Reviews in Meditech, documents in MIDAS, uses Careport for referral process

Required Qualifications

• Valid RN license in Florida
• Minimum 1 year of Case Management experience within the last year
• BLS Certification
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Preferred Skills

• Hospital and/or case management experience required
• Experience with Midas, Interqual, Meditech, and acute hospital case management preferred

Typical Responsibilities

• Patient assessments and evaluations
• Discharge planning with patients and families
• Leading multidisciplinary review (MDR) discussions
• Troubleshooting and applying critical thinking skills

Work Schedule & Expectations

• Scheduled according to facility policy
• Expected to work every other weekend and holidays in rotation with hospital staff
