Job Opportunity: COTA Inpatient Rehab

Location: South Bend, Indiana
Employment Type: Travel/Contract
Contract Length: 13 weeks


Position Details

  • Specialty: Therapy/Rehabilitation
  • Discipline: Certified Occupational Therapy Assistant (COTA)
  • Shift: 5 days a week, 8 hours per day (08:00 - 16:30)
  • Weekend Requirement: One weekend per month (with weekday flexing to avoid overtime)
  • Call Requirement: None
  • Floating Requirement: Yes, floating between Acute and Inpatient Rehab (IPR) units
  • Holiday Requirement: None
  • Guaranteed Hours: 40 hours per week
  • Locals Accepted: No, candidates must live at least 50 miles away from the facility
  • Charting System: Cerner

Required Experience

  • Minimum 1 year of experience in the specialty (COTA)

Licensure & Certifications

  • State License: Pending state license accepted
  • Required Certifications:
    • BLS (Basic Life Support) - AHA only
    • ACLS, PALS, NRP, CHEMO, TNCC, EFM, NIH Stroke Certification, and others may be required depending on unit (refer to unit-specific job descriptions)
    • Tech Certification Verification as applicable per profession
  • Background Check: Required within 90 days of start, covering last 7 years and all states lived/worked in
  • Education Verification: Required (diploma or transcripts)
  • Drug Testing: Negative 12-panel drug screen within 30 days of start
  • Physical Exam: Required
  • TB Testing: Negative 2-step PPD or Qgold within 1 year of start (T-Spots not accepted)
  • Immunizations:
    • Rubeola (Measles), Mumps, Rubella: Titer or 2 vaccines; booster if low titer
    • Hepatitis B: Titer, series, or declination
    • Varicella: Titer or 2 vaccines; booster if low titer
  • Flu Vaccination: Required for flu season (October–May) or approved medical/religious exemption prior to start
  • COVID-19 Vaccination: Medical and religious exemptions accepted with proper documentation

Additional Requirements

  • Must complete facility-specific modules prior to start (approx. 8-10 hours)
  • Facility requires a clear, passport-style photo for badge
  • Confidentiality and Non-Disclosure Agreement must be signed prior to start
  • Candidates must comply with all facility-specific documentation and training requirements

Notes

  • Prior travel experience is not explicitly required for this position.
  • Floating between units requires flexibility and adaptability.
