Required Qualifications

• Valid Mammography Technologist license in Oregon
• Minimum 1 year of Mammography Technologist experience
• Mammography Technologist experience within the last year

Compliance & Documentation Requirements

• Background Check: Must be completed within 45 days of start, covering all locations lived/worked in past 7 years. Includes SSN trace, death master index, nationwide sex offender, FACIS Level 3, education verification, MVR (if applicable), and state-specific abuse/neglect searches.
• Drug Testing: 10-panel drug screen within 90 days of start; instant tests not accepted.
• Health Documents:

  • Physical exam within 1 year of start, signed by MD, NP, PA, or DO.
  • Immunizations and titers: TDAP, MMR (Rubeola, Mumps, Rubella), Varicella, Hepatitis B (or declination), Flu vaccine (seasonal), COVID vaccine or declination.
  • Negative TB test or chest X-ray within 12 months, updated annually.
  • Fit Mask test (N95 or equivalent) required for travel starts after 2/5/2024, updated annually.
  • Respiratory Safety Clearance for travel starts after 2/5/2024, updated annually.
• References: Minimum 2 supervisor references covering at least 1 year of employment in a comparable specialty/setting within the last 3 years.
• Employment Application: Must include at least last 5 years of employment with month/year format; 7 years required for APN/subcontractors.
• Skills Checklist: Required for applicable units, updated annually.

Facility & System Requirements

• Epic Training: Completion of Epic modules required prior to moving to productive work status.
• WorkRamp Modules:

  • Annual completion of system-level regulatory modules required.
  • Specialty-specific and regional modules may be required based on assignment location and specialty.
• Floating Policy: A float policy acknowledgement is required; may be asked to float to other Providence facilities within reasonable distance.
• Photo for Badge: Passport-style photo with neutral background required.

Additional Notes

• No on-call or weekend requirements.
• No 48-hour shifts approved.
• Dress code is flexible (any scrub color or business casual).
• Parking is free.

This position is suitable for Mammography Techs looking for a 13-week travel contract in a Level IV Trauma Center setting with a stable day shift schedule and no weekend or on-call duties. First-time travelers are welcome.
