Registered Nurse - Hospice

Location: Basking Ridge, New Jersey
Employment Type: Travel Nursing

Job Description

• Perform patient assessments to determine appropriateness for hospice care, learning needs, and progress toward goals.
• Develop and implement individualized plans of care for patients and families.
• Maintain visit productivity standards and demonstrate knowledge of insurance reimbursement.
• Complete admission, discharge, and re-visit assessments and documentation within required timeframes.
• Use age-appropriate teaching methods and document patient education tools used.
• Manage case load by prioritizing care needs and revising care plans as necessary.
• Demonstrate knowledge of hospice regulations, insurance reimbursement guidelines, and interdisciplinary communication.
• Perform supervision of home health aides per standards.
• Attend and participate in interdisciplinary team (IDT) meetings every two weeks.
• Maintain a safe environment by following infection control measures and reporting infections.
• Demonstrate knowledge of pharmacology and safe medication management, including medication reconciliation and maintaining current medication orders.
• Participate in performance improvement programs, utilization review meetings, and provide orientation and mentorship to new employees.
• Cross-train to learn new skills and assist with agency growth initiatives.

Contract Length

• Approximately 13 weeks

Required Qualifications

• Valid RN license in New Jersey
• Minimum 2 years of hospice experience, including hospice experience within the last year
• Experience in a home health setting
• BLS certification
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing

Preferred Qualifications

• 3 or more years of RN experience in home hospice

Education

• Associate's Degree in Nursing (ASN) or Diploma with RN License in New Jersey
