Job Opportunity: Registered Nurse - Home Health

Location: Salem, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Provide and coordinate home nursing care according to physician orders and agency policies.
  • Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
  • Conduct home visits with an average patient load of 5-8 patients per day.
  • Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
  • Must have reliable transportation, a current driver’s license, and required liability insurance.
  • Willingness to travel up to 75 miles from the home clinic; average weekly travel approximately 50 miles.
  • Schedule typically Monday-Friday, 8 AM to 5 PM, with some locations offering 4x10 shifts (Friday-Monday).
  • Weekend rotation: approximately 1 weekend per month.
  • On-call requirement: about 1 week per month including weekends (varies by location).
  • Charting and documentation proficiency required in HomeCare HomeBase (HCHB) and OASIS (for RNs).

Required Qualifications

  • Valid Registered Nurse (RN) license in Oregon.
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
  • Minimum of 1 year of recent experience in Home Health nursing, including experience within the last year.
  • Experience working with adult patients in a Home Health setting.
  • Current BLS certification issued by the American Heart Association or American Red Cross.
  • Proficiency with OASIS documentation.

Additional Information

  • Dress code: Scrubs.
  • Productivity expectations: 30 points per week (points assigned based on visits and documentation).
  • Local candidates accepted at the same rate.
  • Background checks, drug screening, and various state-specific compliance requirements apply.
  • Must complete required health screenings and immunizations including physical exam, TB test, MMR, Hepatitis B, Varicella, flu vaccination or declination, and COVID vaccination or declination.
  • References: Two professional references required.
  • Work history verification required for the last 7 years or at least two most recent employers.

Travel Experience

  • Prior travel experience is not explicitly required, but recent specialty experience in home health or hospice is mandatory.
