Job Description

• Physical Therapist needed for Rehab Services with an ASAP start.
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5 days per week).
• Focus on outpatient neuro rehabilitation including vestibular, stroke, and Parkinson’s patients, as well as orthopedic patients.
• Must have recent experience and proficiency with EPIC EMR system.

Required Qualifications

• Valid Physical Therapist license in North Carolina.
• Minimum 2 years of experience as a Physical Therapist.
• Physical Therapist experience within the last year.
• Basic Life Support (BLS) certification.
• Experience with EPIC EMR system.

Compliance and Documentation Requirements

• Proof of I9 or attestation.
• Government-issued photo ID (driver’s license or state ID).
• Background check covering the last 7 years, including all locations lived/worked.
• E-Verify documentation.
• Physical exam updated annually, confirming ability to meet physical demands.
• Negative TB test or chest X-ray within 1 year, updated annually.
• Immunizations or titers within 5 years for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Drug screening (10-panel including marijuana) within 90 days of start, updated annually.
• Seasonal flu vaccination or approved medical/religious exemption required during flu season.
• Two clinical references or hospital evaluations.
• Completion of facility-specific orientation and competency exams.
• Badge photo meeting specific requirements must be submitted within 48 hours of signing.

Additional Notes

• Candidates must complete all required facility-specific documents and orientation materials.
• Criminal history investigation form required if background check returns a hit.
• No candidates residing within 40 miles of the facility will be accepted.
