Position Details

• Specialty: Therapy/Rehabilitation (Physical Therapy - Pelvic Health)
• Shift: Monday to Friday, 8:30 AM - 5:00 PM
• Patient Population: Pelvic floor patients, both male and female
• Caseload: Not specified
• Floating: No floating required
• Weekend Requirement: None
• Block Schedule: Not available, fixed M-F schedule
• Locals Accepted: Yes, no radius restrictions
• On-call: No
• RTO Restrictions: Ideally no more than 5 days off
• Charting System: Cerner (preferred experience)
• Dress Code: Business casual or plain solid scrubs
• Facility Type: Critical Access, Magnet hospital
• Total Staffed Beds: 25

Required Qualifications

• Valid Physical Therapist license in Illinois
• Minimum 1 year of Physical Therapist experience within the last year
• Basic Life Support (BLS) certification

Compliance & Documentation Requirements

• Background checks including criminal, sex offender, and federal exclusions within 30 days of start
• Illinois-specific state searches and registries within 30 days of start
• Fingerprinting must be completed in Illinois 3 business days prior to start (for candidates without IL license)
• Physical exam within 1 year of start, including labs within 7 days of contract signing
• Fit mask test required for new contracts (not completed onsite)
• Negative TB test or chest X-ray within 1 year of start
• Immunizations or documented immunity for Measles, Mumps, Rubella, Hepatitis B, Varicella
• 9-panel drug screen within 30 days of start
• Seasonal flu vaccination or approved medical/religious exemption required annually
• Competency exam and skills checklist required for all clinical candidates
• Two professional references required

Additional Notes

• Must be in Illinois 3 business days prior to start for fingerprinting compliance
• Candidate must review and sign facility-specific documents such as Code of Conduct and Parking Permit Form (if applicable)
• Free parking available in unrestricted spaces
