Minimum Requirements:

• Active Pharmacist license in Ohio required.
• Experience working as a Pharmacist in a hospital setting required.
• Experience with EPIC electronic health record required.
• Experience with medication reconciliation and discharge medication counseling required.
• Clinical experience or demonstrated knowledge in antimicrobial stewardship, including interpretation of culture results and recommendations for common infections (UTI, pneumonia, SSTI) preferred.
• Experience with pharmacokinetic dosing management and anticoagulation dosing management preferred.
• Familiarity with federal and state pharmacy laws, controlled substance handling, inventory processes, and formulary management required.
• Strong communication skills for gathering pre-admission home medication lists and conducting discharge counseling required.
• Minimum 6 months of pharmacist experience preferred.

Assignment Details

• Facility type: Hospital.
• Shift: Day, 5x8.
• Duration: 13 week assignment.
• Candidate will work under general supervision of the Pharmacy Director and collaborate with multidisciplinary teams.
• Responsibilities include sterile and non-sterile preparation, unit dose dispensing, controlled substance management, monthly unit inspections, participation in P&T and medication use review activities, and patient counseling.
• EPIC experience required for order entry, billing functions, and medication reconciliation.
