We're hiring a licensed pharmacist for a contract position where you’ll use your clinical expertise to ensure the safe and thoughtful distribution of medications to individuals navigating a range of physical and mental health challenges. Minimum Requirements:

• Active Oklahoma Pharmacist License required.
• Experience: Minimum 6 months of pharmacist experience preferred.
• Experience with EPIC electronic health record required.
• Medication reconciliation experience for discharge medication lists required.
• Clinical knowledge in antimicrobial stewardship, including interpretation of culture results and recommendations for common infections (UTI, pneumonia, SSTI) preferred.
• Experience with pharmacokinetic dosing management and monitoring required.
• Experience with anticoagulation dosing management and monitoring required.
• Knowledge of controlled substance documentation and inventory procedures required.
• Ability to prepare, label, and dispense sterile and non-sterile products and perform competency assessments for high-volume parenteral preparation required.
• Experience participating in or supporting Pharmacy & Therapeutics, medication use review, and quality assurance activities preferred.
• Strong communication skills for gathering home medication histories and performing discharge counseling required.

• Facility Type: Hospital.
• Shift: Day (5x8).
• Duration: 13 week assignment.
• Float Requirement: Candidate may be required to float to other facilities as needed.
