We're hiring a licensed pharmacist for a contract position where you’ll use your clinical expertise to ensure the safe and thoughtful distribution of medications to individuals navigating a range of physical and mental health challenges. Minimum Requirements:
- Active Pharmacist license for South Carolina required.
- Experience: Minimum 6 months of pharmacist experience preferred. Client preference for experience in hospital or acute care settings.
- EPIC experience required by client.
- Medication reconciliation experience for discharge medication lists required.
- Clinical skills required: antimicrobial stewardship (interpretation of culture results and recommendations for common infections such as UTI, pneumonia, SSTI), pharmacokinetic dosing management and monitoring, and anticoagulation dosing management/monitoring.
- Experience with sterile product preparation, parenteral compounding, and competency assessment of staff who prepare/administer parenterals.
- Experience maintaining controlled substance records and controlled substance handling per federal and state law.
- Strong communication skills for gathering pre-admission home medication lists and conducting discharge counseling with patients and families.
- Ability to perform pharmacy inventory management, procurement procedures, and medication return processes.
- Ability to participate in Pharmacy & Therapeutics and medication use review activities.
- Basic computer and billing system competency as related to pharmacy operations.
- CPR/BLS as required by facility. (Preferred: additional certifications consistent with hospital pharmacy practice.)
