We're hiring a licensed pharmacist for a contract position where you will use clinical expertise to ensure the safe and appropriate distribution of medications to patients across a range of physical and mental health needs. Apply today and a recruiter will reach out to walk you through next steps.

Minimum Requirements:

• Active Pharmacist license in the state of Ohio required to practice.
• Experience in a hospital or acute care pharmacy environment preferred.
• Experience with EPIC or similar electronic medical record systems required.
• Medication reconciliation experience, including preparing discharge medication lists.
• Clinical knowledge of antimicrobial stewardship, including interpretation of culture results and management recommendations for common infections (UTI, pneumonia, SSTI) preferred.
• Experience with pharmacokinetic dosing management and anticoagulation dosing/monitoring preferred.
• Ability to prepare, label, and verify sterile and non-sterile preparations per policy and regulation.
• Knowledge of controlled substance accountability and federal/state pharmacy laws and regulations.
• Strong communication skills for gathering home medication histories and performing discharge counseling.

Preferred Qualifications:

• Six months or more of pharmacist experience preferred.
• Prior involvement in pharmacy quality assurance, medication use reviews, or P&T committee activities preferred.
• Experience training and mentoring pharmacy staff preferred.

Assignment Details

• Shift pattern: 3x8.

Provide high-quality pharmaceutical services that support patient safety, formulary management, medication procurement, inventory control, and clinical medication monitoring. Participate in antimicrobial stewardship, medication reconciliation, staff training, and quality improvement processes.
