We're hiring a licensed pharmacist for a contract position where you’ll use your clinical expertise to ensure the safe and thoughtful distribution of medications to individuals navigating a range of physical and mental health challenges. Minimum Requirements:
- Must have State License to Apply
- EPIC experience
- Medication Reconciliation for discharge medication list
- Communication skills for gathering prior to admission home medication list from patient, family, or pharmacy
- Communication skills for discharge counseling with patients/families
- Clinical knowledge of Antimicrobial Stewardship
- Interpretation of culture results for recommendations
- Knowledge of UTI, pneumonia, SSTI, etc. for recommendations
- Pharmacokinetic Dosing Management/Monitoring
- Anticoagulation Dosing Management/Monitoring
