We're hiring a licensed pharmacist for a contract position where you’ll use your clinical expertise to ensure the safe and thoughtful distribution of medications to individuals navigating a range of physical and mental health challenges. Apply today and a recruiter will reach out to walk you through next steps.

Minimum Requirements:

• Must have State License to Apply
• EPIC experience
• Experience with medication reconciliation for discharge medication lists
• Experience gathering prior-to-admission home medication lists from patients, families, or pharmacies
• Experience with discharge counseling for patients and families
• Clinical knowledge of antimicrobial stewardship
• Experience interpreting culture results for recommendations
• Knowledge of UTI, pneumonia, and SSTI for recommendations
• Experience with pharmacokinetic dosing management and monitoring
• Experience with anticoagulation dosing management and monitoring
