We're hiring a licensed pharmacist for a contract position where you’ll use your clinical expertise to ensure the safe and thoughtful distribution of medications to individuals navigating a range of physical and mental health challenges. Minimum Requirements:

• Must have State License to Apply
• EPIC experience
• Experience with medication reconciliation for discharge medication list
• Communication skills for gathering prior to admission home medication list from patient, family, or pharmacy
• Communication skills for discharge counseling with patients/families
• Clinical knowledge of antimicrobial stewardship, including interpretation of culture results for recommendations and knowledge of UTI, pneumonia, SSTI, and related conditions for recommendations
• Experience with pharmacokinetic dosing management and monitoring
• Experience with anticoagulation dosing management and monitoring
