Job Summary
Cuready Healthcare Staffing is seeking a qualified RN Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.
Responsibilities
- Coordinate care for heart transplant patients, ensuring all aspects of their care are managed effectively.
- Monitor immunosuppression and manage prophylactic antibiotics to prevent complications.
- Conduct routine monitoring for signs of organ rejection and infection.
- Communicate effectively with multidisciplinary teams, patients, and their families regarding care plans and interventions.
- Assist in managing medication regimens and ensure seamless coordination with attending MDs and Pharm D.
- Provide education and resources to patients and families, ensuring comprehensive care support.
Qualifications
- Current CA RN License is required.
- AHA BLS certification necessary for this role.
- Minimum of 1 year experience in ambulatory care settings.
- Specific experience in cardiac nursing, particularly with heart failure or transplant patients.
- Ability to work across various UCSF facilities as needed.
- Preferred knowledge of immunosuppressive therapies and Epic charting system proficiency.
