Job Summary

Cuready Healthcare Staffing is seeking a qualified Registered Nurse - Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

Coordinate and manage patient care activities across the continuum of care.
Engage with interdisciplinary teams to facilitate patient discharge planning and care transitions.
Perform needs assessments and develop individualized plans of care.
Provide education to patients and families regarding care plans and post-discharge instructions.
Utilize EPIC charting system effectively to document patient care activities.
Float between Colby and Pacific Campus to meet patient care needs.

Qualifications

Current RN licensure and BLS certification.
Minimum of 1 year of acute care and discharge planning experience required.
Experience with care coordination in Med Surg, ED, and LLOS preferred.
Proficient in EPIC charting system.
Willingness to float between like units within the hospital.
No locals accepted; candidates must meet the mile radius rule.
