Job Summary

Cuready Healthcare Staffing is seeking a qualified Therapy/Dietary - Physical Therapy to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Manage a caseload of 4-6 patient visits daily using facility's point system.
• Conduct Oasis Assessments and Start of Care cases proficiently.
• Document patient visits and treatments accurately using a tablet device.
• Provide therapy for common cases such as elderly falls and fractures.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Maintain communication with patients and families to discuss care plans and progress.

Qualifications

• Current Physical Therapy License in the state of Indiana.
• Basic Life Support (BLS) certification through the American Heart Association.
• Experience in home health setting preferred.
• Proficiency with Homecare Homebase (HCHB) charting system.
• Ability to work independently and manage multiple tasks efficiently.
• Strong interpersonal and communication skills with patients and healthcare teams.
