Job Summary

Cuready Healthcare Staffing is seeking a qualified RN Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

Coordinate and manage comprehensive patient care plans.
Conduct needs assessments and determine medical necessity using evidence-based guidelines.
Collaborate with healthcare teams to ensure quality care and resource utilization.
Utilize Epic for charting and documentation while adhering to facility protocols.
Facilitate discharge planning activities and ensure continuity of care.
Participate in case reviews to optimize patient outcomes and care efficiency.

Qualifications

Active AK RN License required upon submission.
BLS certification preferred.
Minimum 1 year of acute care hospital experience required.
Familiarity with CMS, ICD-10 coding, and care coordination.
Experience with Epic charting system required.
ACM certification and Microsoft Office/Teams experience preferred.
