Job Summary
Cuready Healthcare Staffing is seeking a qualified RN Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.
Responsibilities
- Coordinate patient care across the continuum, including planning, assessment, and evaluation.
- Ensure accurate and timely case management documentation.
- Manage patient discharges effectively by collaborating with healthcare providers.
- Utilize evidence-based guidelines to determine medical necessity.
- Perform needs assessments and coordinate necessary equipment orders.
- Participate in care plan development and team meetings as required.
Qualifications
- Current AK RN license is required.
- Basic Life Support (BLS) certification preferred.
- Minimum of 1 year of RN experience in an acute care setting.
- Experience with Epic electronic health records system.
- Preferred certification in ACM and experience in care coordination.
- Willingness to work weekends as part of the regular schedule.
