Job Summary

Cuready Healthcare Staffing is seeking a qualified RN - Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities

• Manage utilization of services, ensuring cost-effective care and optimal patient outcomes.
• Coordinate comprehensive discharge planning in collaboration with the healthcare team.
• Ensure integrity of patient revenue by accurate documentation and billing practices.
• Lead and participate in care coordination meetings to enhance service delivery.
• Maintain accurate patient records using EMR systems such as Midas.
• Effectively communicate across disciplines to facilitate patient-centered care.

Qualifications

• Minimum of 2 years of experience in a hospital setting required.
• Active Compact RN license at time of submission required.
• BLS certification required.
• New Hampshire RN license required at time of submission.
• Proven acute hospital RN case management and critical care experience.
• Demonstrated leadership qualities and effective communication skills.
