Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day.- Facility requires primary series COVID vaccination in addition to one booster- Oasis is required. Providers will be responsible for submitting monthly cell phone bill directly to Agency. Modules: Modules are non-billable, average 16 hours spent.
