Provide the following education details for every degree earned: Area of study. Degree type (Associate, Bachelor's, Master's, Diploma, Certificate, etc.). Institution city and state. FOS requires a complete 7-year work history (August 2015 Present), including: Clinical and non-clinical employment. PTO or medical leave. Schooling or other periods not actively working.
