Description:

Licensed Practical Nurse - Full Time Position

Shift Details

• Must be able to start on a Tuesday or Thursday for full shift orientation • Full Time Days and Evening rotations, 5x8 per week • Every other weekend and Holiday required • 40 hours per week

Required Qualifications

• NY LPN Licensure • BLS (AHA or RQI only) • 1 year experience as LPN • High School Diploma or GED • One (1) year experience in LTC or CNA capacity

Position Overview

• Under the direction of licensed nursing personnel, performs direct and indirect designated resident care within the scope of practice as defined by the certification • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age of patients served on assigned unit • Must demonstrate knowledge of the principles of growth and development of the life span and possess the ability to assess data reflective of the patient's status • Must interpret the appropriate information needed to identify each patient's requirements relative to age-specific needs • Provide care needed by adult and geriatric patient group

Job Functions

Resident ADL Care • Meet resident's ADL needs as outlined in the plan of care and in accordance with policies and procedures within a safe and comfortable environment • Includes hygiene, grooming, dressing, incontinence/catheter care, elimination/toileting needs, turning/positioning, skin care • Documentation completed per protocol Environment and Safety • Maintain resident's environment and safety as outlined in the plan of care and in accordance with policies and procedures • Answer call lights and alarms promptly • Provide safety mats and low beds • Restraint use under the direction of Nursing • Maintain cleanliness of unit • Behavior and fall prevention • Appropriate use of and monitoring for malfunctioning equipment • Documentation completed per protocol Nutritional Care • Meet resident's nutritional needs as outlined in the plan of care and in accordance with policies and procedures • Includes meal delivery, tray preparation, adaptive equipment, feeding, nourishments • Honor resident's preferences and choices • Documentation completed per protocol Rehabilitative Care • Meet resident's rehabilitative (restorative and maintenance) needs as outlined in plan of care and in accordance with policies and procedures • Includes range of motion, ambulation, transfers, proper body alignment and use of assistive devices • Documentation completed per protocol Communication and Reporting • Report resident's rights, refusals, and concerns to Charge and/or Supervising Nurse in a timely fashion in accordance with policies and procedures • Identify and report changes in resident physical, emotional and psychosocial status to Charge Nurse promptly

• Occupational exposure to blood borne pathogens is reasonably anticipated • Required tasks involve potential contact with blood, body fluids, or tissues
