Position Summary

This role involves providing comprehensive physical therapy services to clients in a home health setting. The primary responsibilities include delivering direct patient care, collaborating with team members, and actively participating in the development of personalized care plans.

Essential Responsibilities

• Provide direct physical therapy services to patients in their homes.
• Collaborate closely with other healthcare professionals to create and implement comprehensive care plans.
• Utilize various rehabilitation equipment and modalities, including splints, orthoses, prostheses, wheelchairs, and activities of daily living (ADL) equipment.

Position Details

Shift: 8:00 AM - 4:30 PM, 8-hour days with a 40-hour guarantee

Location: Pittsfield, MA

Contract Duration: 91 days

Weekend and Holiday Coverage: Rotating weekends/holiday coverage may be required.

Qualifications

• Home Health Certification required.
• Minimum of 2 years of acute nursing care (hospital-based) and home care experience.
• Valid Driver's License is required.
• Ability to work independently with minimal supervision.
• Acute care experience strongly preferred.
• Proficiency with OASIS is mandatory.

Certifications

• BLS (AHA) certification required.
