Description
Position: Inpatient Case Management Social Worker (LMSW or LCSW)
Location: Anchorage, AK
Shift: Day Shift (5x8 Hours, 08:30 AM - 05:00 PM)
Job Overview
The Inpatient Case Management Social Worker will provide psychosocial and emotional support to patients and their families within an acute hospital setting. The role involves conducting psychosocial assessments, developing discharge plans, and addressing patient care needs through collaboration with the healthcare team.
Key Responsibilities
- Provide brief clinical interventions and resource navigation to patients and families.
- Conduct psychosocial assessments to identify appropriate discharge plans aligned with policy and regulations.
- Screen for suicidality and engage in safety planning as necessary.
- Complete reports of harm according to state statutes.
- Collaborate with RN Case Managers and other healthcare providers to coordinate care.
- Participate in care conference activities and facilitate patient education.
Qualifications
- Master's degree in Social Work from a CSWE accredited school.
- Current Alaska license as LMSW or LCSW (pending licenses will be considered).
- Minimum of 2 years of acute care experience preferred.
Skills and Knowledge
- Knowledge of specialized areas such as adoption, foster care, suicide prevention, and protective services.
- Experience with community-based service programs and resources, including Medicare and Medicaid.
- Strong assessment skills including biopsychosocial assessments and various screening tools.
- Understanding of major disease processes and psychiatric disorders.
- Ability to coordinate comprehensive care plans and document care delivery effectively.
- Experience in various healthcare settings, including emergency departments, inpatient medical care, and outpatient clinics.
Additional Information
This role requires a commitment to providing empathetic support to diverse patient populations, including pediatrics, adults, and geriatrics. The clinician will work closely with multidisciplinary teams to ensure safe and ethical discharge planning.
No travel experience is required, and local candidates are not accepted. Weekend work will be necessary.
Daily Census: 10-20 patients
