Job Description
We are seeking a dedicated professional to evaluate, rehabilitate, and develop effective treatment plans for patients with physical functioning disorders and/or wound care. The successful candidate will adhere to all regulatory and clinical practice standards while following evidence-based practices.
Key Responsibilities
- Conduct screenings and evaluations of patients to develop comprehensive rehabilitation plans.
- Utilize standardized assessments to inform clinical interventions.
- Administer a variety of rehabilitation procedures, including manual techniques, ambulation, therapeutic exercises, and wound care.
- Assess patient progress and adjust treatment plans accordingly, documenting all findings in the clinical record.
- Evaluate treatment outcomes for effectiveness and make modifications as necessary.
- Provide consultation and counseling to patients, families, caregivers, and other service providers.
- Communicate effectively with supervisors and health team members regarding patient progress and treatment plans.
- Prepare comprehensive discharge summaries as per regulatory and clinical requirements.
- Document all required information in the electronic documentation system and maintain accurate patient records.
- Record treatment minutes accurately, adhering to regulatory and clinical standards.
- Participate in required meetings, such as patient care conferences and rehabilitation meetings.
- Engage in facility and company training, in-services, and conferences as required.
- Obtain necessary durable medical equipment (DME) or supplies according to regulatory and clinical guidelines.
- Supervise and support physical therapist assistants, rehabilitation aides, and students as needed.
- Adhere to the American Physical Therapy Association (APTA) and state practice acts, maintaining professional ethics and conduct.
Qualifications
- Minimum of 1 year of physical therapy experience required.
- Experience in long-term care or rehabilitation preferred.
- Current Massachusetts State License required.
