Occupational Therapist

We are seeking a dedicated Occupational Therapist to provide in-home therapy services aimed at enhancing the independence and safety of our home care and hospice patients. This role requires a collaborative spirit as a member of our multidisciplinary team while also being able to work independently.

Key Responsibilities:

• Deliver individualized therapy services in patients' homes.
• Assess patient needs and develop personalized treatment plans.
• Work collaboratively with healthcare professionals to ensure comprehensive patient care.
• Monitor patient progress and adjust treatment plans as necessary.
• Educate patients and their families on therapeutic techniques and safety measures.

Qualifications:

• Valid Occupational Therapy licensure in Vermont.
• Valid driver’s license and access to a reliable vehicle.
• Experience in a home care setting is preferred but not required.
• Ability to work effectively both independently and as part of a team.

Work Schedule:

Monday - Friday

Additional Information:

• Holiday coverage is not required.
• Minimum experience of less than 1 year is acceptable for new graduates.
