Position: Speech and Language Pathologist (SLP) - Long Term Care (LTC)

Shift Notes

8-hour days, 35 hours a week

Job Description

We are seeking a dedicated and skilled Speech and Language Pathologist (SLP) to provide comprehensive care for patients with communication, cognitive, or swallowing disorders in a long-term care setting. As an SLP, you will be responsible for effective screening, evaluation, and rehabilitation, continuously developing appropriate plans of care in compliance with all regulatory and clinical practice standards.

Key Responsibilities:

  • Utilize standardized assessments and evidence-based practices to inform clinical interventions.
  • Assess patient progress and modify treatments accordingly, ensuring documentation aligns with regulatory requirements.
  • Evaluate treatment outcomes for effectiveness, adjusting services as needed, and making referrals based on results.
  • Provide consultation and counseling to patients, along with support for families, caregivers, and other service providers regarding speech disorders.
  • Communicate effectively with supervisors and healthcare team members about patient progress, barriers, and treatment plans.
  • Generate comprehensive discharge summaries according to regulatory and clinical standards.
  • Document all required information in the electronic documentation system, maintaining patient records in accordance with company policy.
  • Accurately record treatment minutes for services provided, ensuring compliance with all regulatory standards.
  • Participate in required meetings, including but not limited to patient care conferences and rehabilitation meetings.
  • Engage in facility and company trainings, in-services, and conferences as mandated.
  • Obtain necessary Durable Medical Equipment (DME) or supplies in alignment with regulatory guidelines.
  • Supervise and support Clinical Fellowship Year (CFY) or Student Speech-Language Pathologists.
  • Adhere to the ASHA Code of Ethics and State Practice Acts throughout professional practice as a Speech-Language Pathologist.

Qualifications:

  • 1 year of SLP experience.
  • Experience in Long Term Care or Rehabilitation settings.
  • Active Massachusetts State License.
  • ASHA Certified and CCC credentials.
  • New Graduates are welcome, provided they have Level II experience rather than just Level I observation.
