We are seeking a dedicated and skilled Physical Therapist to provide comprehensive evaluations and treatments for adult patients in an outpatient setting. The successful candidate will work collaboratively with a multidisciplinary healthcare team to enhance patient functional abilities and overall well-being.

Key Responsibilities

• Conduct general physical therapy evaluations and treatments for diverse age groups with various diagnoses.
• Assess patient needs, perform evaluations, identify therapy diagnoses, formulate treatment plans with goals, and implement therapy services.
• Supervise and modify therapy plans based on patient responses to optimize outcomes.
• Review clinical records and case histories to inform treatment decisions.
• Collaborate with health care professionals and attend care conferences to discuss patient status and needs.
• Document patient encounters, including findings, treatments, diagnoses, and recommendations in accordance with established policies.
• Direct and oversee the work of Physical Therapist Assistants and support staff.
• Develop and implement patient education programs and facilitate home exercise plans, ensuring understanding through feedback and demonstrations.
• Maintain clinical equipment and supplies, adhering to regulatory and quality standards.
• Assist in administrative tasks and represent the Rehabilitation Services Department as required.

Qualifications

• Entry-level qualifying master’s or doctoral degree in Physical Therapy from an accredited program.
• Licensed as a Physical Therapist.
• Current Basic Life Support certification.
• Valid driver’s license.

Experience

• A minimum of 1 year of experience in adult outpatient rehabilitation is required.

Work Schedule

This position typically requires availability Monday through Friday.
