This professional position is responsible for the coordination of outpatient and inpatient services. The role includes assessing, planning, implementing, and evaluating services provided for patients. The Nurse Coordinator serves as a link between the hospital and physicians by providing essential services for both inpatients and outpatients. Responsibilities encompass preadmission assessments, testing, care coordination, and post-procedure assessments.

Key Responsibilities

• Collaborate with interdisciplinary team members to ensure safe and comprehensive patient care in an efficient, cost-effective manner.
• Administer optimal patient care by ensuring the physician has necessary equipment and supplies for procedures.
• Participate in diagnostic and therapeutic procedures in a fast-paced environment.
• Demonstrate accuracy in data collection and patient record updates.
• Maintain a safe and clean environment for patients, visitors, and staff.
• Disinfect and organize equipment and supplies used during procedures.
• Educate patients and families on health care aspects that promote well-being, including reinforcing physician instructions.
• Develop, implement, and monitor standards, policies, and procedures.
• Ensure accountability for information and in-service education disseminated through various platforms.
• Develop and maintain professional relationships while displaying a positive attitude.
• Remain flexible by assuming additional responsibilities as needed.

Qualifications

• Experience: Previous experience as a hospital aide or Licensed Practical Nurse (LPN) is required.
• Licensure: Current NYS Registered Nursing license is mandatory.
• Certifications: Current Basic Life Support (BLS) certification; Advanced Cardiovascular Life Support (ACLS) certification must be obtained prior to the completion of the introductory period.

Physical Requirements

Candidates must possess good dexterity and be able to exert pushing force frequently (10-25 pounds) and occasionally up to 75 pounds. The role also requires carrying (10-15 pounds) and has lifting requirements of up to 50 pounds on rare occasions. Constant walking and standing are required, along with intermittent stooping, crouching, kneeling, and overhead and below-level reaching.
