About the Job
We are seeking a compassionate and independent Travel Occupational Therapist (OT) to provide Home Health services to patients in their residences. In this role, you will evaluate patients in the home environment and deliver personalized, functional therapy focused on improving independence, safety, and performance in daily living activities.
Key Responsibilities

Perform in-home patient evaluations and develop individualized treatment plans

Provide therapy focused on activities of daily living (ADLs), home safety, mobility, and adaptive techniques

Assess patients’ home environments and recommend modifications or assistive equipment
-
Treat patients recovering from surgery, injury, neurological conditions, or chronic illness
-
Monitor patient progress and adjust treatment plans as needed
-
Provide education to patients and caregivers on safety, fall prevention, and home exercise programs
-
Maintain accurate and timely documentation in compliance with home health and regulatory standards

Coordinate care with physicians, nurses, case managers, and interdisciplinary team members
Qualifications
-
Graduate of an accredited Occupational Therapy program
-
Active OT license in the state of assignment
-
Home Health experience preferred

Strong time management, documentation, and independent decision-making skills
-
Reliable transportation and willingness to travel between patient homes
-
Commitment to high-quality, patient-centered care
