Travel Physical Therapist (PT) – Home Health

We are seeking an experienced Travel Physical Therapist (PT) for a home health assignment. The PT will provide in-home therapy services, helping patients improve mobility, function, and independence while managing a flexible caseload.

Assignment Details

• Schedule: Monday–Friday, 8:00 AM – 5:00 PM

• Shift: 5x8 Days

• Length: 13 weeks

• Region: East (Baytown and surrounding areas)

• On-Call: None

• Guaranteed Hours: 40

Responsibilities

• Provide physical therapy services in a home health setting

• Evaluate patients and develop individualized treatment plans

• Deliver therapeutic exercises, mobility training, and functional rehabilitation

• Monitor patient progress and adjust care plans as needed

• Communicate with patients, families, and care teams regarding care plans

• Report missed visits and patient contact issues per protocol

• Maintain accurate and timely documentation using WellSky

Productivity & Travel

• Point system: 1 visit = 1 point

• Weekly expectation: 32–34 points

• Paid drive time and GSA mileage reimbursement

• Drive time is included in scheduling and may qualify for overtime if exceeding 40 hours

Requirements

• Required: Minimum 2 years recent licensed PT experience (no new grads)

• Required: Home health experience

• Required: Active PT license in state of assignment or ability to obtain

• Required: BLS & First Aid certification

• Charting System: WellSky

Additional Information

• Candidates must be prepared to secure housing prior to submission

• Professional attire required (clean scrubs, closed-toe shoes)

• No weekends or holidays unless approved
