13-Week Contract | Day Shift

We’re seeking an experienced Travel Physical Therapist (PT) for a 13-week home health–focused outpatient assignment. This role is best suited for a seasoned traveler who is comfortable managing a mobile caseload and documenting in a home-based care environment.

Assignment Details

  • Shift: Days | Monday–Friday
    (Missed visits may be made up on weekends)

  • Length: 13 weeks

  • Guaranteed Hours (GT): 36 hours/week

  • Setting: Outpatient with approximately 20% Assisted Living visits

  • Patient Load: 6–7 patients per day

  • Charting: WellSky (HomeCare/HomeBase experience required)

  • Travel Radius: Within 50 miles of the facility

  • Mileage: Included in bill rate

  • Expected Weekly Mileage: 50–75 miles (may vary)

  • Facility Work Week: Sunday–Saturday

  • Meetings: Daily stand-up meetings at 8:30 AM (Mon–Fri)

  • Dress Code: Clean, professional scrubs with closed-toe shoes

What You’ll Do

  • Perform patient evaluations and develop individualized plans of care

  • Provide evidence-based PT interventions in outpatient and assisted living settings

  • Manage visit scheduling and complete timely, accurate documentation

  • Participate in daily interdisciplinary stand-up meetings

  • Educate patients and caregivers on mobility, safety, and recovery

Requirements

  • Active Physical Therapist license

  • Minimum 2 years of recent licensed PT experience (required)

  • At least 1 recently completed travel assignment (required)

  • Home health experience with HomeCare/HomeBase (required)

  • BLS & First Aid certifications (required)

  • Strong organization, communication, and time-management skills

  • No new graduates

Additional Requirements

  • COVID-19 vaccination and boosters required
    (Medical/Religious exemptions reviewed case-by-case; approved exemptions require weekly testing provided by the facility)

  • Housing must be secured prior to submission
