Description

We are seeking an experienced Travel Physical Therapist (PT) for a home health assignment in South Carolina. The PT will provide skilled in-home therapy services, managing a patient caseload within an assigned territory to restore function and improve quality of life.

Home Health experience is REQUIRED – candidates without this will not be considered.

Assignment Details

Start Date: ASAP or 06/01/2026
Length: 13 weeks
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
On-Call: None
Setting: Home Health (field-based)
Territory: Assigned geographic area (case management model)
Location: Ladson, SC
Bill Rate: $95/hr (firm)
Mileage: Reimbursed at IRS rate

Responsibilities

Provide direct, hands-on physical therapy services in patients’ homes
Evaluate patients and develop individualized treatment plans
Deliver therapeutic interventions to improve mobility, strength, and functional independence
Supervise and direct care provided by Physical Therapist Assistants (PTAs) and Home Health Aides
Coordinate care with physicians, families, and interdisciplinary teams
Maintain accurate and timely documentation in the electronic medical record
Manage a daily caseload within assigned territory

Requirements

Required: Minimum Home Health experience
Required: Active South Carolina PT license
Required: American Heart Association (AHA) BLS Certification
Required: Valid driver’s license and ability to travel between patient homes
Required: Graduate of an accredited Physical Therapy program
