Description

We are seeking an experienced Travel Occupational Therapist (OT) for a 13-week assignment in an outpatient setting. This role primarily serves outpatient patients, with approximately 20% of visits conducted in Assisted Living Facilities. The OT will evaluate patients, develop individualized treatment plans, and provide therapy services focused on improving independence, functional abilities, and quality of life.

Assignment Details

Schedule: Days, Monday–Friday
Missed visits may be made up on weekends as needed
Length: 13 weeks
Setting: Outpatient with some Assisted Living Facility visits (approximately 20%)
Guaranteed Hours: 40 hours per week
Facility Work Week: Sunday–Saturday
Travel Radius: Up to 50 miles from the facility
Expected Weekly Mileage: Approximately 50–75 miles (may vary)
Mileage Reimbursement: Included in bill rate
Patient Ratio: Approximately 6–7 visits per day
Charting System: WellSky
Daily Stand-Up Meeting: Required Monday–Friday at 8:30 AM

Responsibilities

Evaluate patients and develop individualized occupational therapy treatment plans
Provide therapeutic interventions to improve activities of daily living (ADLs), mobility, coordination, and functional performance
Deliver therapy services in outpatient settings and Assisted Living Facilities as assigned
Monitor patient progress and modify treatment plans as needed
Educate patients and caregivers on adaptive techniques and home programs
Maintain accurate and timely documentation in WellSky
Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care

Requirements

Required: Graduate of an accredited Occupational Therapy program
Required: Active OT license in state of assignment or ability to obtain
Required: Current BLS and First Aid certifications
Required: Minimum 2 years of recent licensed OT experience
Required: Minimum 1 recently completed travel assignment
Required: Experience with HomeCare/HomeBase
Required: Home health experience
No New Graduates

Additional Requirements

COVID-19 vaccination and booster(s) required
Medical and religious exemptions considered on a case-by-case basis
Approved exemptions require weekly testing per facility policy
Candidates should review housing options prior to submission
