Description
We are seeking a compassionate and dedicated Physical Therapist Assistant (PTA) to join our rehabilitation team in a Skilled Nursing Facility (SNF). Working under the supervision of a licensed Physical Therapist, the PTA will provide high-quality therapy services designed to help patients improve mobility, increase strength, restore function, and maximize independence.
This is an excellent opportunity to work alongside a collaborative interdisciplinary team while making a meaningful impact on the lives of patients recovering from illness, injury, surgery, or other medical conditions. The ideal candidate is patient-focused, adaptable, and committed to delivering exceptional, evidence-based care.
Responsibilities
- Implement individualized treatment plans developed by the supervising Physical Therapist
- Provide therapeutic exercises, gait training, balance activities, transfer training, strengthening exercises, and functional mobility interventions
- Monitor patient progress during treatment sessions and communicate changes or concerns to the supervising Physical Therapist
- Educate patients and caregivers on home exercise programs, mobility techniques, fall prevention, and safety strategies
- Maintain accurate and timely documentation in accordance with facility policies, Medicare guidelines, and regulatory standards
- Collaborate with Physical Therapists, physicians, nurses, occupational therapists, speech-language pathologists, and other members of the interdisciplinary healthcare team
- Assist with discharge planning by providing updates on patient progress and recommendations for continued therapy
- Maintain a safe, clean, and organized treatment environment
- Deliver compassionate, patient-centered care focused on improving mobility, restoring function, and enhancing quality of life
Requirements
- Required: Graduate of an accredited Physical Therapist Assistant program
- Required: Active Physical Therapist Assistant (PTA) license in the state of practice or ability to obtain
- Preferred: Previous experience in a Skilled Nursing Facility (SNF) or post-acute care setting
- Strong communication, documentation, organizational, and interpersonal skills
- Ability to work effectively as part of a multidisciplinary healthcare team
