Description
We’re seeking a motivated and compassionate Travel Physical Therapist (PT) for a 13-week assignment in a Skilled Nursing / Post-Acute Care setting. This role offers the opportunity to work with a geriatric population, collaborate with an interdisciplinary rehab team, and make a meaningful impact on patient mobility and independence.
Assignment Details
Position Type: Travel / Contract
Length: 13 weeks
Schedule: Day shift
Guaranteed Hours: Typically 36–40 hours/week
Setting: Skilled Nursing Facility (SNF)
Start Date: ASAP or upcoming start dates available
What You’ll Do
Evaluate patients and develop individualized plans of care
Provide skilled PT interventions focused on strength, balance, gait, and functional mobility
Monitor patient progress and adjust treatment plans as needed
Accurately document evaluations, treatments, and outcomes
Collaborate with nursing and rehabilitation teams
Educate patients and caregivers on mobility, safety, and home exercise programs
What You Need
PT or DPT from an accredited program
Active or eligible Physical Therapist license
SNF or post-acute experience preferred (new grads may be considered, depending on facility)
Strong communication, time-management, and teamwork skills
Commitment to patient-centered care
