Description

Occupational Therapist (OT) – Skilled Nursing Facility (SNF) 

We are seeking a skilled and compassionate Occupational Therapist (OT) to join a collaborative therapy team within a skilled nursing facility setting. This position focuses on providing high-quality rehabilitation services to a primarily geriatric population, including short-term rehab patients and long-term care residents. 

The OT will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions designed to restore functional independence, improve safety, and enhance overall quality of life. This role requires strong clinical judgment, efficiency, and the ability to thrive in a fast-paced, productivity-driven environment. 

Key Responsibilities: 

  • Perform comprehensive occupational therapy evaluations and establish individualized plans of care 

  • Provide skilled treatment targeting ADLs, functional mobility, strength, balance, cognition, and safety awareness 

  • Implement fall prevention strategies and environmental modifications 

  • Recommend and train residents in adaptive equipment and compensatory techniques 

  • Complete accurate and timely documentation, including evaluations, progress notes, recerts, and discharge summaries 

  • Collaborate with interdisciplinary team members including PT, SLP, nursing, and case management 

  • Supervise and direct COTAs in accordance with state practice guidelines 

  • Maintain productivity standards while ensuring patient-centered care 

Ideal Candidate: 

  • Active OT license in practicing state 

  • SNF or geriatric experience preferred 

  • Strong clinical reasoning and time management skills 

  • Comfortable managing both short-term rehab and long-term care caseloads
