Description
We are seeking an experienced Speech-Language Pathologist (SLP) for an 8–13 week Home Health travel assignment in the Houston region. The SLP will provide skilled speech therapy services to patients in their homes, including evaluations, treatment, patient and caregiver education, and ongoing assessment of patient progress.
Candidates must have at least 2 years of recent licensed SLP experience and previous Home Health experience. This may be the candidate's first travel assignment; however, new graduates will not be considered.
Assignment Details
- Setting: Home Health
- Region: Houston
- Contract Length: 8–13 weeks
- Schedule: Monday–Friday, 8:00 AM–5:00 PM
- Hours: 5x8-hour day shifts / 40 hours per week
- Weekends/Holidays: No weekends or holidays unless approved in advance for a scheduled visit
- On-Call: None
- Guaranteed Hours: 40
- Charting System: WellSky
- Mileage: GSA maximum rate
- Required Certifications: BLS and First Aid
Responsibilities
- Perform speech-language pathology evaluations in patients' homes and develop individualized plans of care.
- Provide skilled treatment for speech, language, communication, cognitive, and swallowing disorders as appropriate.
- Establish treatment frequency and goals based on evaluation findings and patient needs.
- Educate patients, families, and caregivers on treatment strategies and home programs.
- Monitor patient progress and modify interventions as clinically appropriate.
- Complete accurate and timely documentation using WellSky.
- Communicate with physicians, nursing staff, therapists, and other members of the interdisciplinary care team.
- Call in all missed visits, evaluation frequencies, and instances when a patient cannot be contacted.
- Travel throughout the assigned Houston region to provide scheduled home visits.
- Maintain professional standards of care and comply with all agency policies and procedures.
Productivity & Travel Expectations
- Visits are based on a point system of 1 point per standard visit.
- Expected productivity is 32–34 points per week.
- Candidates are compensated for both drive time and GSA mileage.
- Drive time is considered when scheduling and calculating workload.
- If drive time results in the clinician exceeding 40 hours for the week, overtime is paid.
- No Start of Care (SOC) visits are currently expected. If SOC visits are added, they would most likely be valued at approximately 3 points.
Requirements
- Current and unrestricted SLP license in the applicable state.
- Minimum 2 years of recent licensed SLP experience required.
- Private-duty experience does not count toward the required 2 years.
- Previous Home Health experience required.
- No new graduates.
- First-time travelers are welcome provided they have qualifying Home Health experience.
- Current BLS and First Aid certifications required.
- Strong communication, organization, time-management, and documentation skills.
- Ability and willingness to travel between patients' homes throughout the assigned region.
- Ability to work independently in a Home Health environment.
Dress Code & Additional Information
Clinicians must wear clean, professional scrubs with closed-toe shoes. Facial jewelry is not permitted except for a small nose stud and earrings.
