Job Opportunity: Home Health Physical Therapist (PT)

Job Details

• Start Date: ASAP (Compliance process takes about 3 weeks)
• Shift: 8-hour day shifts
• Patient Ratio: 1:1 (May visit up to 6 patients per day)
• Typical Caseload: 4-5 adult patients per day
• Weekend Requirement: None
• On-Call Requirement: None

Experience & Licensing Requirements

• Minimum 1 year of recent skilled home health PT experience within the last 3 years (inpatient experience not accepted)
• Must hold a valid Washington state PT license
• BCLS (Basic Cardiac Life Support) certification required
• OASIS documentation experience required

Additional Information

• Own transportation is required; system groups clients to minimize drive time
• Occasional travel radius up to 100 miles depending on territory
• Timely communication and documentation are mandatory
• Documentation tools will be provided and must be returned

Required Certifications & Compliance

• BLS (Basic Life Support)
• HIPAA Safety Manual Acknowledgement (annual)
• State License Verification (within 90 days of start and upon expiration)
• Background Check (within 90 days of start, updated annually) including SSN trace, county searches, nationwide checks, motor vehicle records, education and work history verification
• Drug Testing: 9-panel drug screen within 12 months of start
• Physical Exam: Physician statement within last 12 months
• TB Testing: Negative TB test or chest X-ray within required timeframes; annual TB questionnaire required
• Immunizations: Measles, Mumps, Rubella, Hepatitis B, Varicella (vaccines, titers, or declination accepted)
• COVID-19 Vaccination or approved exemption (dose 1, fully vaccinated, and booster)
• Seasonal Flu Vaccination or declination for 2025-26 season
• Competency Exams as applicable to profession
• Employment Application with full 7 years of work history (no gaps)
• Professional References (2 for travel/allied/per diem acute care; 1 for per diem non-acute care) within 2 years of start
• Fingerprinting requirements for Washington candidates who have not lived in the state for the past 3 years (completed on-site)

• Proof of current auto insurance required due to driving responsibilities
• Facility requires completion of Acceptable Use Policy and Agency Background Attestation prior to start
• Documentation and compliance requirements are strictly enforced to meet Joint Commission and facility standards
