Job Opportunity: Home Health Occupational Therapist (OT)
Location: Spokane Valley, Washington
Facility: Compassus Providence Spokane Home Health
Hospital System: Allfaith Homecare
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Start Date: ASAP (Compliance process takes about 3 weeks)
- Shift: 8-hour day shifts
- Patient Ratio: 1:1 (May visit up to 6 patients per day)
- Productivity Standard: 27-30 productivity points based on territory
- Weekend Requirement: None
- On-Call Requirement: None
- Locals Accepted: Yes
- Case Load: Adult patients, 4-5 patients per day
- Experience Required:
- Minimum 1 year of recent skilled hospice experience within the last 3 years
- Inpatient experience not accepted
- Certifications Required:
- BCLS (Basic Cardiac Life Support)
- Licenses Required:
- Washington State OT license
- NBCOT certification
- OASIS documentation required
Additional Information
- Transportation: Traveler must have their own transportation
- Territory: Occasional travel radius up to 100 miles; system groups clients to minimize drive time
- Documentation: Timely communication and documentation required; documentation tools provided and must be returned
- Attire: Any color scrubs allowed

Compliance & Onboarding Requirements
Certifications
- BLS (Basic Life Support) - AHA, ARC, Military acceptable
- HIPAA Safety Manual Acknowledgement (annual)
- State License Verification (primary source within 90 days of start and upon expiration)
- NURSYS Verification (for RN, LPN/LVN, AP as applicable)
Competencies
- Medication Test/Pharmacology Competency Exam (as applicable)
- Competency Exam (as applicable to profession/specialty)
Facility Specific Documents
- Proof of current auto insurance (required for all Home Health positions involving driving)
- Acceptable Use Policy (must be completed prior to start)
- Agency Background Attestation (required for all candidates)
Health Documents
- Physical exam within last 12 months
- Negative TB test or chest X-ray (within 1 year; chest X-ray within 5 years if positive PPD)
- TB Questionnaire (annual renewal required)
- Immunizations or titers for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B
- Varicella
- Drug testing: 9-panel drug screen within last 12 months
- Seasonal flu vaccination or declination (2025-26)
- COVID-19 vaccination or medical/religious exemption (including booster)
Background & History Checks
- Background check within 90 days of start, updated annually, including:
- SSN trace
- County searches (last 7 years)
- FACIS Level 1
- Nationwide sex offender check
- Motor vehicle records
- Education and work history verification
- EPLS/GSA/SAM check within 90 days of start
- Two professional references within 2 years of start
- Employment application with full 7 years of work history (no gaps)
- Skills checklist (annual)
- Fingerprinting (WA candidates who have not lived in WA for past 3 years; completed on-site)
- State-specific criminal history and registry checks as applicable
