- MGB National Sex Offender Check (within 30 days of start, updated annually)
- MGB Exclusion Checks (within 30 days of start and annually; includes OIG, GSA, MassHealth & NH Medicaid lists)
Certifications & Health Requirements
- BLS (AHA, Red Cross, CHSF, or Military; AHA preferred)
- ARRT certification
- Fit Test (N95 respirator fit test within 365 days of start, updated annually; no religious exemptions)
- MMR Vaccination or Proof of Immunity (2 measles, 2 mumps, 1 rubella vaccine or 2 MMR vaccines, or blood test proof)
- TB Test (skin test or blood test within 3 months of start, updated if >6 month break)
- Health Statement (physician statement within 365 days of start, updated if >6 month break)
- Respirator Medical Clearance Attestation (part of fit test record)
Optional but Preferred
- Hepatitis B vaccination or proof of immunity (strongly preferred)
- Influenza vaccine (required during flu season, Sept 15 - April 30)
- Tdap vaccine (within 10 years, preferred)
- Positive PPD with Negative Chest X-Ray documentation (if applicable)
- TB Symptom Survey (if positive PPD, updated annually)
