Job Opportunity: Home Health Physical Therapist (PT)
Location: Lacey, Washington
Specialty: Therapy/Rehabilitation
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Details
- Start Date: ASAP (Compliance process takes about 3 weeks)
- Shift: 8-hour day shifts
- Patient Ratio: 1:1 (May visit up to 6 patients per day)
- Productivity Standard: 27-30 productivity points based on territory
- Weekend Requirement: Possible, to be discussed during interview
- On-Call: No call required
- Locals Accepted: Yes
Experience & Licensing Requirements
- Minimum 1 year of recent skilled Home Health PT experience within the last 3 years (inpatient experience not accepted)
- Must have a valid Washington state PT license
- BCLS (Basic Life Support) certification required
- OASIS documentation required
Caseload & Work Environment
- Adult patient caseload, approximately 4-5 patients per day
- Traveler must have their own transportation
- Client grouping minimizes drive and windshield time, with occasional travel radius up to 100 miles
- Timely communication and documentation are required
- Documentation tools will be provided and must be returned

- Type of Facility: Home Health
- Attire: Any color scrubs allowed
- Charting System: Home Care Home Base (required)
- Parking: Free
- Mileage Reimbursement: IRS standard
Compliance & Background Requirements
- Background checks must be current within 90 days of start and include:
- SSN Trace
- County searches for last 7 years of residence/work/school
- FACIS Level 1
- Nationwide Sex Offender Registry
- Motor Vehicle Records/Driver’s Record
- Education and Work History Verification
- EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
- Fingerprinting required for candidates who have not lived in Washington for the past 3 years (completed on-site)
- State-specific criminal history and employability status checks as applicable
- Education verification of highest degree related to profession required
Certifications & Competencies
- BLS (Basic Life Support) certification required (AHA, ARC, Military accepted)
- Orientation Safety Manual Acknowledgement (updated annually)
- State license primary source verification within 90 days of start and upon expiration
- Medication Test/Pharmacology Competency Exam as applicable
- Competency exams as applicable to profession/specialty
Health & Immunization Requirements
- Physical exam within the last 12 months (not required annually)
- Fit test for N95 mask required
- Negative TB test or chest X-ray within 1 year (with exceptions for positive TB history)
- Annual TB questionnaire required
- Immunizations or proof of immunity for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B
- Varicella
- 9-panel drug screen within 12 months of start (instant acceptable)
- Seasonal flu vaccination or facility-specific declination form required during flu season
- COVID-19 vaccination or medical/religious exemption required, including booster or declination form
Additional Requirements
- Proof of current auto insurance required due to driving responsibilities
- Completion of facility-specific Acceptable Use Policy prior to start
- Two professional references within 2 years of start required
- Full 7 years of work history with no gaps must be documented and verified
This position is suitable for therapists with recent home health experience who are licensed in Washington and able to meet comprehensive compliance and health requirements.
