Job Opportunity: Home Health Occupational Therapist (OT)
Location: Lacey, Washington
Specialty: Therapy/Rehabilitation
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Details
- Start Date: ASAP (Compliance process takes about 2 weeks)
- Shift: 8-hour day shifts
- Patient Ratio: 1:1 (May visit up to 6 patients per day)
- Productivity Standard: 27-30 productivity points based on territory
- Case Load: Adult patients, 4-5 patients per day
- Weekend Requirement: Possible, to be discussed during interview
- On-Call: No call required
- Locals Accepted: Yes
- Attire: Any color scrubs
- Charting System: HomeCare HomeBase (required)
- Parking: Free
- Mileage Reimbursement: IRS standard
Experience & Certification Requirements
- Minimum 1 year of recent skilled hospice experience within the last 3 years (inpatient experience not accepted)
- Must hold a valid Washington state OT license and NBCOT certification
- BCLS (Basic Life Support) certification required
- OASIS documentation required
Additional Notes
- Own transportation is required
- Client grouping minimizes drive and windshield time; territory radius may be up to 100 miles
- Timely communication and documentation are mandatory
- Documentation tools will be provided and must be returned
Compliance & Background Requirements
- Background checks must be current within 90 days of start and include:
- SSN Trace
- County searches for last 7 years
- FACIS Level 1
- Nationwide sex offender check
- Motor Vehicle Records (MVR)
- Education verification
- Work history verification (7 years)
- EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
- Fingerprinting required for candidates who have not lived in Washington for the past 3 years (completed on-site)
- State-specific criminal history and employability status checks as applicable
Certifications & Competencies
- BLS (AHA, ARC, Military accepted)
- Orientation Safety Manual Acknowledgement (updated annually)
- State license primary source verification within 90 days of start and upon expiration
- Medication test/pharmacology competency exam as applicable
- Competency exam as applicable to profession/specialty
Health & Immunization Requirements
- Physical exam within the last 12 months (not required annually)
- Fit test for N95 mask (no specific model required)
- Negative TB test or chest X-ray within 1 year (chest X-ray within 5 years if positive PPD)
- Annual TB questionnaire required
- Immunizations or titers for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B
- Varicella
- 9-panel drug screen within last 12 months (instant acceptable)
- Seasonal flu vaccination or facility-specific declination form required during flu season
- COVID-19 vaccination or medical/religious exemption required, including booster or declination form
Documentation & Other Requirements
- Proof of current auto insurance required due to driving responsibilities
- Completion of facility-specific Acceptable Use Policy prior to start
- References:
- 2 professional references for travel, allied, per diem acute care
- 1 professional reference for per diem non-acute care
- References must be within 2 years of start date
