Location: Newton, Massachusetts
Employment Type: Long-Term Assignment (LTA)

Job Summary

  • Provide physical therapy services to patients in their homes with physical impairments, functional limitations, or disabilities due to injury, disease, or other causes.
  • Patient age range: birth to over 100 years.
  • Collaborate with patients, families, physicians, and allied health professionals to identify needs and coordinate services.
  • Maintain thorough and timely electronic documentation and interdisciplinary communication per agency policies and standards.
  • Weekend rotations occur once every 6 weeks.
  • Ability to travel to assigned cases is required.

Requirements

  • Must hold a valid Physical Therapist professional license in Massachusetts (MA Conditional Licenses not accepted).
  • Minimum 1 year of home care experience required.

Contract Length

  • 13 weeks

Credentialing and Onboarding Requirements

Professional Credentials

  • Physical Therapist License/Certificate Verification: Primary source verification within 30 days of start and updated upon expiration.
  • MGB Badge Photo: Recent color photo (within 6 months), no selfies or filters, plain background, symmetrical headshot, no eyeglasses.
  • MGB MVR Check: Motor vehicle record check within 45 days of start, required for all home care positions.
  • MGB Criminal Background Check: Within 45 days prior to start, updated if break >6 months. Includes international searches if applicable.
  • MGB Massachusetts CORI: Criminal history check for MA within 30 days prior to start, updated if break >6 months.
  • MGB National Sex Offender Check: Within 30 days of start, updated annually.
  • MGB Exclusion Checks: Within 30 days of start and annually; includes OIG, GSA, MassHealth, and NH Medicaid exclusion lists.
  • MGB Education Verification: Verification of highest education level through background check company.
  • MGB SSN Trace: Run with criminal check, listing addresses/aliases for past 7 years.
  • MGB Resume: Submission required with past 7 years of work experience including city and state.
  • MGB References: Two recent references required, at least one supervisor or above in skillset.
  • MGB Skills Checklist: Required for submission.
  • MGB Candidate Submission Coversheet: Required for submission.
  • MGB Confirmation of Assignment/POI Form: Required for all initial assignments and extensions upon acceptance.

Personal Health History and Vaccinations

  • Fit Test (N95 Respirator): Within 365 days of start, updated annually. Must be completed on MGB form; no religious exemptions.
  • Respirator Medical Clearance (RMC) Attestation: Required if fit test is required.
  • MMR Vaccine or Immunity Proof: Documentation of two measles, two mumps, and one rubella vaccine or two MMR vaccines, or proof of immunity via blood test.
  • Hepatitis B: Strongly preferred but optional. Record of 3 vaccines, immunity proof, or declination accepted.
  • Influenza Vaccine: Required during flu season (Sept 15 - April 30). No exemptions allowed.
  • TB Test: Documentation of TB skin test or negative QuantiFERON/T-spot blood test within 3 months of start, updated if >6 month break.
  • Positive PPD - Negative Chest X-Ray: If TB test positive, chest x-ray documentation required.
  • Health Statement: Physician statement signed by MD/DO/NP/PA within 365 days of start, updated if >6 month break.
  • Tdap Vaccine: Preferred within last 10 years, optional.
  • MGB Nurse Aide Registry Check: Required within 30 days of start for all employees per MA DPH.

Additional Notes

  • No religious or medical exemptions for fit testing or influenza vaccine.
  • All documentation must be current and compliant with Massachusetts Department of Public Health regulations.
  • Travel to patient homes is required as part of the role.
  • Weekend work is infrequent (once every 6 weeks).

This position is ideal for a licensed Physical Therapist with home care experience who is comfortable working independently in patient residences and meeting comprehensive credentialing and health requirements.
