Job Summary

Responsible for regulatory compliance, utilization review, chart audits, and performance improvement in a home health setting.

Key Responsibilities

• Implement and evaluate performance improvement, quality, and utilization activities.
• Conduct reviews of medical records (plans of care, clinical and progress notes) for quality and compliance.
• Facilitate interdisciplinary case conferences, monitoring coordination of care and OASIS data.
• Monitor OASIS data integrity and complete OASIS data submission.
• Ensure development, implementation, and updates of individualized plans of care.
• Produce monthly reports to monitor quality of services and publicly reportable data.
• Assist with evaluation of clinical competence and compliance.
• Stay informed about home health rules, regulations, and standards.
• Ensure compliance with regulatory standards in home health service delivery.
• Promote a clean and safe environment using SAFE error prevention habits.
• Maintain highest standards of privacy and confidentiality.

Required Qualifications

• Minimum 2 years of home health experience.
• Previous quality experience.
• COS-C (Certificate for OASIS Specialist-Clinical) certification.
• Valid Illinois nursing license.
• Valid driver’s license and car insurance.
• OASIS experience.
• COVID vaccination or approved declination.

Work Details

• Territory: Within 15 miles (may increase if census is low)
• Case load: 24-26 patients (may decrease with larger territory)
• Mileage reimbursed at standard IRS rate
• Some telehealth/virtual visits in addition to home visits
• EMR system: Matrix (iPad provided)
• Orientation: 2 days (1 day in office, 1 day in field)
• Interview process: Phone screen plus in-person or virtual interview
